What forms do I need to complete for my Child(ren)'s registration?

Each participant must be verified upon registration and by the first day of practice with the following information and supporting documents:

- copy of birth certificate (give to Head Coach on or before first day of practice)

- proof of address or school district (viewed/checked by Head Coach)

- Active Braves Nation Member Account (registration fees paid or current)

- Health and Medical Waiver

- Fit for Play First Day Form

- Participant Code of Conduct Agreement

- Parent (PGV) Code of Conduct Agreement

If your child has a medical condition (asthma, allergies, etc.) that requires medication, please take notice of our Secure Meds Policy.

All forms for athletic participation can be found on our website under BravesNation menu tab and then select Forms in the dropdown menu.  Please complete, print and sign all forms and provide a copy to your Head Coach on the first day of practice. Thank you.

We are working to streamline online submission and storage of forms so that you will not need to print these forms in the future.  For now, however, please provide a copy of these completed forms to your Head Coach.
