Fingerprint Background Check: 
1)  Link to online scheduling for your fingerprinting appointment: www.bioapplicant.com/nj
2)  You will need to have a copy of the most recent Universal Fingerprinting Form as a reference in order to complete your appointment registration.
3)  Before you go to your appointment, make certain the Form has not been updated or revised since making your appointment.
4)  If you do not have the correct form you will be turned-away without reimbursement of your appointment fee, and then you do the online scheduling again as well as pay the fee again.
5)  Be sure to read all the notices on the scheduling page, changes to forms and procedures are made rather frequently.
6)  Box 7 of the Form should be filled-in with the code U1200. If it is not, you will need to add that code to the Form.
7)  Payment is required at time of scheduling. You will need your receipt, proper ID, and competed Form with your for your appointment (or they will turn you away).
8)  If you’d like to process a request reimbursement of your fingerprint fee from NBYFL, please complete our Expense Request Form online and upload/attach a copy of your receipt
9)  When you are done with your appointment, SEND a copy of your receipt to Shelley at admin@nbyfl.com as verification of your appointment and so she can log the date of your appointment (for future tracking purposes).
10) The Fingerprinting service will not provide duplicate receipts, PCN Numbers or appointment information after time of printing.
